The Department of Nursing requires supplemental staff to work at least 208 hours per year. This position would see patients in the Southeast Minnesota geography including the towns of Red Wing, Rochester, and Cannon Falls. Mileage is reimbursed. The Hospice Patient Care Coordinator (Registered Nurse) is accountable for the delivery of nursing services based on initial and ongoing assessments of patient needs and in accordance with the interdisciplinary team plan of care. The delivery of nursing services includes direct care, patient/family/caregiver support, instruction and education. In addition, the RN provides telephone triage and on-call responsibility 24/7. The ANA Standards of Clinical Nursing Practice and the Standards of Practice for Hospice Programs provide a basis for the practice of the RN in hospice. The RN supports professional nursing practice across practice settings by providing holistic, family-centered care in efforts to improve quality of life. Qualifications. Student or graduate of an accredited, or those in the candidacy process, baccalaureate, associate or diploma nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Exceptions to this may be made in the MCHS sites prior to a law change requiring accreditation expected in 2018. If graduated from a nursing program that was not accredited by ACEN, CCNE, or NLN CNEA, at least one year of RN experience (within the last 5 years) in an applicable care setting is required. If graduation did not occur within the last two years, one year of RN experience in an applicable care setting is required, or one year of current LPN experience at Mayo Clinic is required. All associate degree or diploma registered nurses hired into the Department of Nursing with an RN start date after April 1, 2020, must provide documented evidence of program completion of their baccalaureate degree in nursing from an accredited nursing program within 5 years from the last day of the month of their RN start date. Additional qualifications. Minimum of two years' previous RN experience in acute care, home care and/or long-term care settings preferred. Bachelor's degree with a major in nursing preferred. Hospice experience preferred. Possesses strong verbal and written communication skills. Ability to adapt to unpredictable situations within the home setting. Computer and keyboarding skills preferred. Internal applicants should submit their three most recent performance appraisals. License or certification. Must obtain prior to start date:Current RN license by applicable state requirements. Basic Cardiac Life Support for healthcare providers- BCLS - certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.
